Interwell Health's Exceptional Performance
Over the last three years, Interwell has emerged as the largest participant in the Comprehensive Kidney Care Contracting (CKCC) program, generating $273 million in shared savings for the government. This performance not only reflects significant financial success but also highlights Interwell’s commitment to high-quality care, achieving an average quality score of 88%, which is nine points above that of other CKCC participants. In 2024, Interwell leads the initiative by representing 44% of high performers, earning the highest number of perfect scores in the program's history.
Interwell Health's financial performance is characterized by a staggering 43% higher average adjusted gross savings compared to organizations that are not part of its network. The company has also demonstrated continuous improvement by increasing its gross savings by one percentage point per year, all while providing care to a record 59,520 patients across 23 Kidney Care Entities (KCEs) as of 2024.
A Patient-Centered Approach
The success can largely be attributed to Interwell’s patient-centered approach and its integrated use of advanced technology, which together empower patients and lead to improved health outcomes. CEO Tommy O’Connor emphasizes these elements as central to the company's strategy, which seeks to enhance care coordination for Medicare patients dealing with chronic kidney disease (CKD) and end-stage renal disease (ESRD).

The KCC Model itself focuses on enhancing quality metrics, and Interwell has consistently excelled in areas such as optimal dialysis starts, increased utilization of home-based therapies, and higher Patient Activation Measure (PAM) scores. Interwell Health’s achievements not only indicate its dominance in the field of value-based kidney care but also demonstrate a transformative impact on patient outcomes, supporting a substantial 71% growth in shared savings rates for the healthcare system.
